Comments:
An unannounced monitoring inspection was done today 3/28/2018 beginning at 9:50 AM and ending at 2:30 PM. There were 69 children in care today not including the school age children. The children were observed during their activities such as playing , eating snack, art, lunch and swimming. Medication was reviewed. Six children and five staff's record were reviewed. This included newly enrolled children and hired staff. Discussion with director: infant room changes, allergy care plans, finger prints, and summer camp.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation of the playground, the center failed to ensure that areas and equipment inside and outside of the center was maintained in a clean, safe operable condition. Evidence: 1. Observed on the playground the cement edge over by the large school age slide did not have a covering and had a sharp edge.

Plan of Correction: The director work with the maintenance people to have the sharp point covered.

Standard #: 22VAC40-185-340-F
Description: Based on observation the center failed to ensure that all children under 10 year of age were always within actual sight and sound supervision of staff. Evidence: 1. Observed staff # 6 walking out of the staff lounge after getting a soda from the vending machine, walking back into the toddler classroom. There were 15 toddlers napping. Staff # 6 left the children alone and out of sight and sound supervision while getting a drink. The staff lounge is located across the hall from the toddler's classroom.

Plan of Correction: The director will speak with the staff person and will monitor all staff.

Standard #: 22VAC40-185-510-J
Description: Based on observation, the center failed to ensure that all medications were locked. Evidence: 1. Observation of the infant room the medication for child # 7 was unlocked and not accessible to the children in care.

Plan of Correction: The medication was moved locked closet.

Standard #: 22VAC40-185-560-F
Description: Based on observation, the center failed to ensure that when food is provided that all components are served according to the standard. Evidence: 1. The children served cereal and and water. The menu stated that cereal with apple juice would be served. The requirement is for two components to be served.

Plan of Correction: The director will make sure that the menu is followed at every meal.

Standard #: 63.2-1720.1-B-2
Description: Based on review of five records, the center failed to ensure that all staff that started work after January 22, 2018 had finger prints completed and returned prior to starting the position. Evidence: 1. The record record for staff # 1 has a documented start date of 2/14/2018 and the finger print letter was received on 3/15/2018.

Plan of Correction: The director will from now on will make sure that all new hires will have the finger print letter.
