We discussed the requirements for TB statements and children's records. The risk assessments for violations was provided. We talked about the changes with background check requirements.
Thank you for your assistance during the unannounced monitoring inspection conducted from 10:55 AM to 3:00 PM. Today, there were sixteen children, ages three through five years, with two to three teachers. I viewed program activities, daily routines, equipment, staffing, supervision, interactions with children, posted information, emergency supplies and drill records, asbestos checks, five records for children, five staff records, staff training, meals/snacks, indoor and outdoor areas. The children enjoyed outdoor activities, library time/books, music, group discussions and choices for play in classroom interest areas. Creative art activities have been offered. The staff were observed facilitating learning experiences and supporting children's choices for appropriate behavior. Let me know if you have any questions. 540/430-9259
Standard #: 22VAC40-185-140-A Description: Based on a review of five records for children, and interviews with administrative staff, the school failed to obtain a copy of the physical examination record for child 2. Evidence: There was only an immunization report in the record. The physical examination report form was blank other than the child's identifying information at the top. Plan of Correction: A staff person immediately contacted a parent and was told the parent will arrange to have the signed physical examination report faxed from the physician's office. In the future, the staff who accept records for children will view the forms to make sure all required information has been received before forms are filed.
Standard #: 22VAC40-185-160-C Description: Based on a review of five records for staff, and an interview with administrative staff, the updated TB screenings and statements for staff member 1 and 2 were not obtained by two years from the dates of the most recent statements. Evidence: The most recent statement in the record for staff member 1 was completed on November 4, 2015. The most recent statement for staff member 2 was completed on January 23, 2015. Plan of Correction: An administrative staff person will request an updated TB statement for a current screening from staff member 1 and 2. The records will be viewed and placed in the files. An administrator will regularly check a chart with dates for staff records to make sure that arrangements for TB screenings are made in advance of due dates and that all updated information is obtained on time.
