Technical Assistance:
Discussed appropriateness of transitioning a child to an older age group classroom. Documentation that is required in order to transition a younger child to an older classroom is allowable if the child is determined age and stage appropriate for the older class.

An unannounced monitoring inspection was conducted this date. A total of 79 children present with 15 staff. Observation of all 10 classrooms was made. Children were observed sitting in a circle for book time, learning about jungle animals and transitioning to outside playtime. Both infant rooms observed, walk thru and evaluation of the cribs was made. Infants were observed being offered tummy time, sleeping in cribs and being held while being fed. The center was decorated with seasonal themed artwork that the children had completed. Foot and hand print painted clovers were on display along with hand drawn artwork. Lunch today was chicken, rice and apples. A total of 6 staff records and 7 children's records were reviewed today. Fingerprinting of new staff was verified. Thank you for your cooperation today. Description: Based on child record review, a health exam was not maintained for one child. Evidence: Child #4 began care on 10/2/17. No health exam was found for this child.

Plan of Correction: Director will email parents today to request a health exam.

Standard #: 22VAC40-185-130-A
Description: Based on child record review, immunizations were not obtained prior to enrollment for one child. Evidence: Child #6 began care on 1/2/18. Immunizations maintained in the record is dated 1/19/18.

Plan of Correction: Will ensure that all immunizations are received prior to the child's first day.

Standard #: 22VAC40-185-210-A
Description: Based on staff record review, two staff do not meet lead teacher qualifications. Evidence: Staff #2 and #5 are designated lead teachers. Both staff do not meet the lead qualifications by receiving 24 hours of training within their first 6 months of hire.

Plan of Correction: We will re train and reorganize the documentation of lead training. We will have both these staff receive a total of 24 hours of training to qualify as leads. One staff will also provide a copy of their education.

Standard #: 22VAC40-185-270-A
Description: Based on observation of the physical plant, areas of the building are not maintained in a safe condition. Evidence: The following areas were found not maintained in a safe condition: Areas of the carpet in the World Market, Club House and Think Tank classrooms are torn in some areas presenting a tripping hazard. Carpet wall coverings in the Pathfinders and World Market classrooms are coming off the walls. Carpet located on the step in the World Market classroom has torn and exposing bare wood on the edge of the step. Peeling paint was found in the Trailblazer classroom within reach of children in care.

Plan of Correction: Will contact home office regarding repairs or replacements for the carpeted areas. The paint will be repaired.

Description: Based on observation of the physical plant, one bathroom did not provide soap for hand washing. Evidence: The bathroom located in the Art Studio class did not have hand soap.

Plan of Correction: We will place hand soap in this bathroom immediately.

Standard #: 22VAC40-185-520-C
Description: Based on observation of the physical plant, diaper ointment was found accessible to children. Evidence: Diaper ointments were located in an unlocked cabinet within reach of the children in care in the Trailblazer and Inventor's Workshop.

Plan of Correction: Secured the ointments immediately and will remind staff to lock these at all times.
