An unannounced monitoring inspection was conducted today from 10 - 1:30 pm. There were 55 children present at the beginning of the inspection in 6 classrooms during spring break. I observed children busy with a variety of activities including crafting masks, making handprints, involved in a pretend bug hunt, crafting baby "chicks," and building with manipulatives.questions, please contact me If you have any questions, please contact me at firstname.lastname@example.org
Standard #: 22VAC40-185-260-A Description: Based on a review of records today and staff confirmation, there was no current annual fire inspection available. Evidence: The last inspection was dated 9/9/16. Plan of Correction: We have called several times. They are coming 6/20/2018.
Standard #: 22VAC40-185-270-A Description: Based on observation today, some outdoor areas were not maintained in a safe and operable condition. Evidence: There were cracks in the resilient poured surface under the larger play structure, posing a tripping hazard, especially at the bottom of the slide chute. This was cited 12/17. There was bare netting on the toddler playground grassy surface, posing a tripping hazard where sod had been placed and was no longer covered. There were some areas of splintering and rusty bolts on the wooden picnic tables. Plan of Correction: I will send this report to maintenance today. We will block off areas where the sod has worn away.
Standard #: 22VAC40-185-330-B Description: Based on observation today, there were some bare surface areas in the falls zones of the smaller play structure while a substantial amount of mulch was present in areas away from the play structure. Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 22VAC40-185-510-A Description: Based on a review of children's medications today, and observation of the administration of a long-term medication, the center did not follow the center's written medication policies. Evidence: When Child # 2 came to the office for an as-needed long-term medication, there was an inadequate amount of medicine in the over-the-counter bottle to give to the child to provide relief. The center did not contact the parent when the remaining medication was running low. Plan of Correction: I already called mom to bring a new bottle, and I will retrain MAT trained staff to be aware of meds running low.
Standard #: 22VAC40-185-510-G Description: Based on observation today, a child's medication was present without the prescription label as required. Evidence: A single dose of inhaler medication was present for Child # 4, but no prescription label was present. Plan of Correction: I will talk to the parent today.
