Discussed acceptable CPR and First Aid certifications.

Comments:
An unannounced renewal inspection was initiated today. The physical plant, 2 staff files, 5 children's files, staff training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected between 11:15 and 12:30. There were 9 children under the supervision of 2 staff. During the inspection, children were observed going outside, playing outside on the playground and leaving for the day. For snack, the children were served cheese sticks, apples and water. The interactions between the children and staff were seen to be positive and constructive in nature. Both staff are certified to administer medications. However, there are no children with any medication needs. Should you have any questions, please contact me at mary.wizbicki@dss.virginia.gov.

Description: Based on file review, 1 out of 2 staff records does not have a TB test/screening completed within the past two years. Evidence 1. Staff #2's most recent TB test/screening was completed on 3/9/16.

Plan of Correction: This will be completed again.

Standard #: 22VAC40-185-60-A
Description: Based on file review, 1 out of 5 children's records does not have the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached Evidence 1. Child #4 only has one emergency contact on file.

Plan of Correction: We will ask the parents for another emergency contact.
