22VAC40-191 Background Checks (22VAC40-191)

An unannounced Monitoring Inspection was conducted on April 4,2018 from approximately 10:10 am until 2:15 pm. Upon entrance there were 17 children and 3 staff present with children. Additional staff and children arrived during the inspection. The record review sample consisted of 5 children's records and 4 staff records. Children were doing stretches, exercises and eating snack. The center is no longer administering medication. The infants were observed freely playing. The results of the inspection were reviewed and verified with the center director/owner. Description: Based on documentation review and staff interviews, the licensee did not ensure the injury prevention procedures were updated at least annually based on documentation of injuries and a review of the activities and services. Evidence: During the inspection on April 4, 2018, there was not an updated injury prevention plan. The center staff confirmed it had not updated within the last year.

Plan of Correction: The center director updated during inspection and will update it in the future annually.

Description: Based on observation, staff interviews and record review, the licensee did not ensure that for children birth to the age of 16 months there was one staff member for every four children. Evidence: During the inspection on April 4, 2018, 16 children age range 3- 11 were observed in the large multipurpose room with three staff . At 10:42 am two additional children age 16 months, entered the room and remained in the multipurpose room until 11:18 am. There were 18 children, age range 16 months to age 11 with 3 staff and the required ratio of one staff with four children was not met for 36 minutes.

Plan of Correction: The center director will make the ratio is upheld at all time.

Description: Based on staff interviews and documentation review, the licensee did not ensure a menu listing foods to be served for meals and snacks during the current one-week period was posted or a copy given to the parents Evidence: During the inspection on April 4, 2018, the menu posted was not dated for the current week.

Plan of Correction: The center director will make sure the correct dates are put on the menu.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on a record review of 4 records and staff interviews, the licensee did not ensure that before three years since the dates of the last criminal history record check report (CRC) an updated check had been conducted. Evidence: During the inspection on April 4, 2018, the record for staff #1 included the most recent CRC check dated 1-13-2015.

Plan of Correction: The center director will make checks are completed on time and will obtain an updated check and place it in the file.
