Discussed with center management the requirements for mandated reporting of suspected child abuse and neglect within 24 hours.
An unannounced complaint inspection was conducted in response to a complaint that was received by the licensing office on 3/13/18. Staff interviews were conducted with staff regarding allegations of forbidden actions of staff flicking a child on mouth with her fingers. The information gathered during this investigation supports the allegations, so the complaint is determined to be valid. Standard #: 22VAC40-185-410-1 Complaint related: Yes Description: Based on staff interview, staff used forbidden actions towards children in care. Evidence: 1. Staff # 1 stated that on 3/9/18 she saw Staff # 2 "flick" Child # 1 on the upper lip. Child # 1 is two years of age. 2. Staff # 1 stated that on 3/8/18 she observed Staff # 2 "bop" Child # 2 on the back of his head with her hand. Staff # 1 demonstrated what she meant by "bop", as similar to a slap on the back of the head. 3. Staff # 1 stated that on 3/8/18 she had also seen Staff # 2 "flick" Child # 3 on the lips with her finger. Child # 3 is two years of age. Plan of Correction: Upon immiediate notification of this to Director, Staff # 2 was put on leave and subsequently terminated effective 3/9/18.
