Comments:
An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted from 7:00 am - 8:30 am. There were 28 children in care being supervised by 4 staff members. The children were observed participating in table top activities and eating breakfast. Breakfast served today is Egg and cheese sandwich, pear, cereal bar, and milk. Ten children's records were reviewed and in compliance. Four staff records were reviewed and in compliance. An allergy list was posted for children enrolled in the program with food allergies. Four children's records were reviewed for Allergy Care Plans for children enrolled in the program with diagnosed food allergies. Five medications were reviewed. There are three staff members on site who are MAT (Medication Administration Training) certified. All staff on site are CPR and First Aid certified and have obtained the Virginia Pre-Service Training. Description: Based on review of three staff records and interview with administration it was determined that an allergy care plan was not on file for all children enrolled in the program with food allergies. Evidence: There was no documentation of an Allergy Care Plan on file for Child A and Child B who have diagnosed food allergies.

Plan of Correction: Administration will request and allergy care plan from the parents of Child A and Child B.
