Comments:
An unannounced monitoring inspection was conducted on 03/27/2018 from 12:02pm until 1:30pm. Upon arrival to the facility, there were 56 children present with 14 staff members. The children were observed hand washing, eating lunch and napping. A sample of 5 children's records and 5 staff records were reviewed. The licensing inspector toured the facility and reviewed supervision, emergency supplies, practice drills, nutrition, physical plant, required postings, and administration. Violations were cited in the areas of physical plant and administration. An exit meeting was conducted with the facility director prior to the end of the inspection.

Description: Based upon observation and interview the provider did not ensure that the electrical outlets located inside of the facility had protective covers to prevent potential injuries of children. Evidence: 1. During the inspectors tour of the facility it was found that an electrical outlet located in the preschool room of the facility did not have a protective cover. a. The electrical outlet without a protective cover could be a potential hazard to children. b. There were children in the preschool room during the time the violation was observed. c. The staff #5 toured the preschool room with the inspector and also identified and confirmed the violation.
