Standard #: 22VAC40-185-160-C
Description: Based on record review, a repeat tuberculosis screening was not submitted at least 2 years after initial screening. Evidence: Staff B, hired 10/15/2015 last completed a tuberculosis screening 10/4/2015. An subsequent screening had not been initiated.

Plan of Correction: Teacher returning completed TB documentation.

Standard #: 22VAC40-185-270-A
Description: Based on observation, equipment was not maintained in operable condition. Evidence: The trash can located near the changing table in the 2 year old classroom wasn't operable.

Plan of Correction: Trash receptacle will be replaced.

Standard #: 22VAC40-185-290-3
Description: Based on observation, protective covers were not used on electrical outlets. Evidence: 1) In the pre-k room , two outlets near the boys restroom did not have protective covers. 2) In the early preschool classroom, one outlet near the entrance, one out let near the boys restroom and one outlet on the wall near the fire extinguisher were missing protective covers. 3) In the 2 year old classroom, 1 outlet above the sink and 1 outlet near the entrance of the room were missing protective covers.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-185-440-E
Description: Based on observation, occupied cribs were not at least 12 inches apart. Evidence: 1 occupied crib in the infant room was found in the middle of the room with other 2 other cribs blocking the sides if the occupied crib.

Plan of Correction: Teachers have been retrained.

Standard #: 22VAC40-185-540-A
Description: Based on observation, the first aid kit was incomplete. Evidence: The first aid kit was missing 2 of 2 triangle bandages.

Plan of Correction: Fabric was cut into two squares and folded into triangles. Corrected during inspection.

Standard #: 22VAC40-185-550-M
Description: Based on record review, minor injury logs were incomplete. Evidence: Of 7 minor injury logs reviewed the following was missing: 1) 4 minor injury logs were missing the date, time and method in which parent was notified of the minor injury.

Plan of Correction: Teachers and Managers will be sure information is on incident reports.

Standard #: 22VAC40-191-40-C-1-B
Description: Based on record review and interview, a Central Registry Search was not resubmitted after 45 days of employment. Evidence: Staff A, hired 5/31/2017 did not have a Central Registry Search on file. The search was not repeated after 45 days of employment . The director stated that the search needed to be re-sent.

Plan of Correction: Teacher returning notarized statement to be sent. 4/4/2018.

Standard #: 63.2-1720.1-B-3
Description: Based on record review, out of state Central Registry Searches were not initiated. Evidence: Staff A, hired 5/31/2017, currently lives outside of the state of Virginia and a Central Registry Search was not been initiated in the current state of residence.
