Description: Based on records reviewed, a TB screening was not performed at least every two years after initial test. Evidence: 1) Staff A's TB test/screening was last conducted 08/09/2015. 2) Staff B's TB test/screening was last conducted 07/15/2015.

Plan of Correction: Staff records will be located/updated.

Description: Based on record review, children's records were incomplete. Evidence: Of 7 children's records reviewed, the following was missing: 1) Child A was missing an annual review, file was last reviewed 08/2014. 2) Child B was missing 2 of 2 emergency contact phone numbers and an annual review. File was last reviewed 11/2016. 3) Child C was missing fathers work phone number. 4) Child D was missing mother's employer, fathers employer phone number and an annual review. File was last reviewed 07/2015. 5) Child E was missing the address of 1 of 2 emergency contacts and an annual review. File was last reviewed 12/2014. 6) Child F was missing father's employer phone number.

Plan of Correction: Parents will be reminded to provide updated/complete record info.

Description: Based on observation, hazardous substances such as cleaning materials were not kept in a locked place. Evidence: 1) In the "Toddler's Room", disinfectant spray and all purpose cleaner was found unlocked on a shelf in the bathroom. 2) In the "Lil Montessori Room", disinfectant spray, all purpose wipes, and all purpose cleaning spray was found unlocked on a shelf in the bathroom. 3) In the "Montessori 1 Room", disinfectant wipes were found in an unlocked cabinet on the wall. 4) In the "Montessori 2 Room", disinfectant wipes were found in an unlocked cabinet on the wall.

Plan of Correction: Items will be placed in locked container.

Description: Based on observation, food brought from home was not clearly dated and labeled in a way that identifies the owner. Evidence: 5 lunch boxes in the "Toddler's Room"; 5 lunch boxes in the "Lil Montessori Room"; 4 lunch boxes in the "Montessori 1 Room" and 7 lunch boxes in the "Montessori 2 Room" were not dated and labeled with the name of the owner.

Plan of Correction: Staff will tape label food bags from home.

Description: Based on record review, central registry findings were not returned within 30 days of employment. Evidence: Staff C hired 09/14/2017, did not have central registry findings on file.
