- Technical Assistance:
The following areas of CDC standards were discussed with the administrator: +Program leader and program director qualifications; +Time frames for out-of-state central registry checks; +Attendance documentation to include children of staff; +Completion of injury reports.
An unannounced renewal inspection was conducted on 03/29/2018 from 9:45am - 1:05pm. At the time of entrance there were 13 children in care with four staff. The children ranged in age from six months - six years. Children were observed during outdoor play. Lunch was served outside at the picnic tables. Nap was additionally reviewed. The sample size included eight children and seven staff records. No medication is being administered at this time. The violations are listed on the violation notice and were reviewed with the administrator at the conclusion of the inspection.
Standard #: 22VAC40-185-160-A Description: Based on record review and interview, the center failed to ensure that documentation of the TB screening shall have been completed within 12 months prior to or 21 days after employment. Evidence: 1. The TB screening on file for staff 1 was completed on 08/07/2016. a. Staff 1 has a hire date of 02/12/2018 and was working during the inspection. 2. Administrative staff and staff 1 confirmed this was the only TB screening on file. Plan of Correction: Will update staff file immediately.
Standard #: 22VAC40-185-560-G Description: Based on observation, the center failed to ensure that when food is brought from home the food container shall be clearly dated. Evidence: Four of eight lunch containers reviewed were not dated. Plan of Correction: We will ensure all lunch boxes are properly dated.
Standard #: 22VAC40-185-570-E Description: Based on review, the center failed to ensure that prepared infant formula shall be dated and labeled with the child's name. Evidence: 1. Four bottles containing breast milk were not dated. a. Three of these bottles were not labeled with the child's name. 2. All of the above bottles belonged to the same child according to the infant teacher and center administrator. Plan of Correction: We will ensure all bottles are labeled properly.
