Technical assistance was provided in the following areas of the standards: 22VAC40-191 (Background checks); 22VAC40-185-(4)-270-A (Building maintenance); 22VAC40-185-(4)-330-B (Resilient surfacing); 22VAC40-185-(7)-510- (Medication)
An unannounced monitoring inspection was conducted on 4/3/18 from 10:25am - 1:15pm. When the Licensing Inspector arrived there were 91 children ages two months old to ten years old in care with 16 staff. Children were observed participating in various activities in the classrooms, playing outside, eating lunch and during rest period. Records were reviewed for nine children and nine staff. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.
Standard #: 22VAC40-185-130-B Description: Based on a review of nine staff records, it was determined that the facility did not ensure that additional immunizations are obtained once every six months for children under the age of two years old. Evidence: 1. The record for child #4 (two years old) contained an immunization record that was dated 8/1/17, which is from when the child 13 months old. 2. Staff #10 (Program Director) reviewed the record for child #4, and confirmed that additional immunizations records were not obtained every six months for child #4 while he was under the age of two years old. Plan of Correction: The facility responded: The parent of the child without the updated immunization record was contacted immediately upon visit for an updated shot record. Management will continue to remind parents that whenever their child receives new immunizations, they are responsible for submitting copies of them to the office.
Standard #: 22VAC40-185-440-J Description: Based on observation it was determined that the facility did not ensure that there shall be at least twelve inches of space between the sides and ends of occupied cribs. Evidence: 1. There was an occupied crib in the Infant room where there was less than twelve inches on both ends of the crib. 2. Staff #10 (Program Director) confirmed that the occupied crib did not have twelve inches of space on each end. Plan of Correction: The facility responded: Occupied cribs were immediately pulled apart to reflect the 12 inches of required space between the ends of cribs. Staff were retrained on 4-3 on the appropriate spacing of cribs.
Standard #: 22VAC40-185-550-M Description: Based on a review of the injury reports from the facility, it was determined that the facility did not ensure that the center maintains a written record of children's serious and minor injuries that includes all of the required items. Evidence: 1. Four of the injury reports from March 2018 reviewed did not include the future action to prevent recurrence of the injury. 2. Staff #10 (Program Director) reviewed the four injury reports and confirmed that the four in jury reports did not include all of the required items. Plan of Correction: The facility responded: Staff were retrained on 4/3 as to the importance of determining the best action to be taken to prevent recurrence on an incident report. Going forward, when management signs off on all incident reports, they will check to ensure that that portion of the incident report has been completed.
