A Subsidy Health and Safety inspection was conducted on 4/3/18 from 10:25am to 1:15pm to review supplemental health and safety requirements. At the time of entrance there were 91 children ages two months old to ten years old in care with 16 staff. Children were observed participating in various activities in the classrooms, playing outside, eating lunch, and during rest period. Records were reviewed for nine children and nine staff . A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: VENDSUB-000-005-B Description: Based on observation and interviews, it was determined that the vendor did not ensure that for each group of children, the vendor shall maintain a written record (hard copy) of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: 1. The attendance record for the School Age classroom indicated that there were 13 children present. However, the Licensing Inspector counted 15 children present in the classroom 2. Staff #10 reviewed the attendance record and confirmed that the arrival of all the children present was not documented on the attendance record. Plan of Correction: The vendor responded: The attendance record was updated immediately so it included all oft he children present in the classroom. Staff will be trained to record the arrival time and departure time of each child on the daily attendance sheet to ensure it is documented. Center management will double check attendance sheets on a daily basis to ensure that the arrival time and departure time is recorded for each child.
