Comments:
An unannounced monitoring inspection was conducted with the director beginning at 3:30 PM and ending at 4:50 PM. There were eleven school-aged children in the care of three staff members during the inspection. The children were observed playing Jeopardy as a large group. Six children's records were reviewed. Two staff records were reviewed and in compliance. Thirteen medications were reviewed. The emergency escape drill was reviewed and in compliance. The most recent emergency escape drill was documented as 3/19/18. The most recent shelter-in-place drill was documented as 9/15/17. The most recent lockdown drill was documented as 1/18/18. The most recent health inspection was documented as 5/8/17. The most recent fire inspection was documented as 2/8/17. Violations:
Standard #: 22VAC40-185-130-A
Description: Based on a review of documentation, the center did not obtain documentation of immunizations for all children before attendance. Evidence: Child C (start date 8/28/17) did not have documentation of immunization in their record.

Plan of Correction: The center will request a copy of the child's immunization record and upon receipt it will be placed in the record.

Standard #: 22VAC40-185-260-A
Description: Based on a review of documentation, the center did not have documentation of an annual fire inspection being conducted as required. Evidence: The most recent fire inspection was documented as 2/8/17.

Plan of Correction: The center director will notify school staff that the fire inspection is expired. Once an updated fire inspection is conducted the documentation will be filed.

Standard #: 22VAC40-185-510-E
Description: Based on a review of medications, not all of the medications were labeled with the child's name, the dosage amount, and the times to be given. Evidence: Child A and B's had medications that were not labeled with their names, the dosage amount, and the times to be given.

Plan of Correction: The center will notify the parent that each of the medications need to be labeled with the child's name, dosage amount, and time to be given.

Standard #: 22VAC40-185-510-J
Description: Based on observation, not all of the medications were kept in a locked placed using a safe locking method that prevents access by children. Evidence: Child A and B had emergency medication stored in a fanny pack around their waste during the inspection.

Plan of Correction: The center will collect the medication from the children upon entering the program and they will be kept in a locked placed inaccessible to children.
