Technical Assistance:
Have six months of supervised programmatic experience. a. Within six months before being promoted or beginning work or one month after being promoted or beginning work, a minimum of 12 hours of training shall be received related to the care of children, including but not limited to: (1) Child development; (2) Playground safety; (3) Health and safety issues; and (4) Preventing and reporting child abuse and neglect

Comments:
An unannounced monitoring inspection was conducted this date. A total of 151 children with 15 staff were on site. Walk thru of all 7 classrooms was conducted. Observation of programming was made. Classrooms were decorated with seasonal artwork which consisted of hand painted clovers, tissue paper rainbows and counting skittles worksheet. Children were observed being read to by staff using an animated voice which the children were actively engaged to. Classrooms equipped with tactile zippers, locks and latches within reach of the children so they could independently participate on their own. Majority of classrooms were completing morning activities in small group activities which consisted of table top puzzles, magnetic letters, magic marker drawing. Observation of bathroom procedures, hand washing and bottle feeding was made. All medications were reviewed and authorizations determined complete. All topical ointments were current and labeled. Staff observed sitting with the children while eating their lunches. A total of 8 children's records and 5 staff records were reviewed. Previous violations from the last licensing inspection were reviewed and corrective actions were verified. Thank you for your cooperation today. Standard #: 22VAC40-185-110-A
Description: Based on child record review, 3 out of 8 records reviewed did not have documentation of a health exam maintained. Evidence: Child #1 began care on 9/11/17, no documentation of a health exam was found. Child #3 began care on 3/20/17, no documentation of a health exam was found. Child #5 began care on 2/6/17, no documentation of a health exam was found.

Plan of Correction: We will ensure that all health exams are obtained and maintained within 30 days of enrollment.

Standard #: 22VAC40-185-130-A
Description: Based on child record review, three out of 8 records reviewed did not have documentation that immunizations were obtained prior to enrollment. Evidence: Child #4 began care on 7/3/17. Documentation of immunizations was dated 7/12/17. Child #5 began care on 2/6/17, no documentation of immunizations was found. Child #7 began care on 4/4/16, no documentation of any immunizations was found.

Plan of Correction: We will ensure that immunizations are obtained before any child begins care.

Standard #: 22VAC40-185-130-B
Description: Based on child record review, documentation for updated immunizations every six months was not found. Evidence: Child #6's last documentation of immunizations was dated 12/2016. Child #8's last documentation of immunizations was dated 3/2016. Immunizations are required every six months until the child is 2 years of age.

Plan of Correction: We will request documentation of updated immunizations for all children under two years of age every six months.

Standard #: 22VAC40-185-70-A
Description: Based on staff record review, 4 out of 5 records reviewed were incomplete. Evidence: Staff #1, 2 and 3's records did not contain documentation that references were obtained prior to hire. Staff #2,3 and 4 are designated lead teachers. No documentation was found that these teachers qualify to be in a lead teacher role.

Plan of Correction: We will ensure that all new hires will have documented reference checks prior to hire.

Standard #: 22VAC40-185-270-A
Description: Based on observation of the physical plant, one area of the building was not maintained in a safe condition. Evidence: The corner located near the carpet area in the Pre K 2 classroom had exposing drywall and carpet that is no longer adhered to the wall.

Plan of Correction: We will begin with an estimate and will repair this area.

Standard #: 63.2-1720.1-B-3
Description: Based on staff record review, background checks were determined incomplete. Evidence: Out of state Central Registry Searches were not completed for two staff who were determined to have lived out of the State of Virginia in the past five years.

Plan of Correction: Will request these searches for both staff.
