An unannounced renewal inspection was conducted today from 10am through1:30pm. There were 95 children present with 12 teaching staff. Children present ranged in age from three to five years old. Children's records were reviewed. Staff emergency contact information was reviewed. Staff records are stored at a central location and will be reviewed at a later date. Children were observed in teacher and self-directed activities including circle time, learning center play activities, transitions, restroom routines, hand washing, lunch and nap. Violations regarding physical plant and medication was observed and appear on the violation notice.
Standard #: 22VAC40-185-270-A Description: Based on observation and interview the center failed to ensure that areas are maintained in operable condition. Evidence: 1 - There are buckets placed on the floor in classrooms C and F to collect water leaking through the ceiling from the roof. 2 - Staff present in classrooms C and F stated that the roof leaks after it rains. However, they are never sure when the leaking will begin. Sometimes it is two days after a rain that the ceiling leaks. Plan of Correction: Center management will place a work order indicating that the leaks need to be addressed as soon as possible.
Standard #: 22VAC40-185-510-G Description: Based on record review and interview the center failed to ensure that prescription drugs are administered only in accordance with the prescriber's instructions pertaining to dosage, frequency, and manner of administration. Evidence: 1 - The prescription drug Benadryl for child 1, has a prescription label that reads; 8ml for allergic reactions every 4 - 6. a. However, the log sheet has hand written across the top "to be administered daily at 12noon". b. The signed authorization states "before nap time and/or before going outside." 2 - Staff 2 stated that the "mom" requested that the medication be administered at noon each day. Plan of Correction: Center management will ensure that the medication authorization and prescription labels match. Center management will remind and retrain all staff that the administration of medication must always follow the doctor's orders/prescription and that a parent may not request alterations of medication administration instructions without a written notification from the doctor/prescription.
