Comments:
The Licensing Inspector conducted an unannounced investigation on 03/23/2018, and a follow up was conducted on 03/29/2018 in response to a complaint that was received by the licensing office on 03/22/2018 regarding special care provisions, special services and programs. Based upon multiple staff interviews, records reviewed, and observations made, the preponderance of evidence gathered during this investigation does not support the allegations. Description: Based on observation, areas and equipment of the center, were not maintained in a safe and operable condition. Evidence: In the Daffodils classroom, peeling paint was observed on the wall under a classroom window, next to the entrance door.

Plan of Correction: Area in question has been sanded and re-painted.
