An unannounced monitoring inspection was completed on 4/5/18. There were 67 children present in seven groups with 12 direct care staff supervising and two administrative staff in addition were present. Five children's records and five staff records were reviewed. According to the site coordinator, there were no medications at the center for any children in care. The inspector reviewed staff records at the Central site at 10:45 am. The inspection at the facility began at 12:45 pm and ended at 4:30 pm. The children were observed during free time in the classrooms, during nap, during afternoon snacks, during homework time for the school-age children, and during gym time. There was discussion with the administrator about the following: lock down procedures and drill practices, injury prevention annual update, policies and procedures to be reviewed annually at inspections, documentation of program lead qualifications when staff have completed the requirements.
Standard #: 22VAC40-185-240-C Description: Based on record review, the center failed to ensure that staff who work directly with children attend at least 16 hours of staff development training annually. Evidence: 1. There was no documentation of any training hours for staff #3 for the 2017 year. Staff #3 was hired Each staff is required to attend 16 hours of staff development training annually. Plan of Correction: The site coordinator and director will ensure staff attend at least 16 hours annually and document all training.
Standard #: 22VAC40-185-560-K Description: Based on observation and interview, the center failed to ensure that the tables used for feeding were sanitized before and after as required. Evidence: 1. The school-age afternoon snack was observed. The children were observed to go directly from eating snack and washing their hands to doing homework on the same tables used for snack. The tables were not sanitized after use for snack. 2. According to interview with the school-age lead staff person, the practice is that the tables are sanitized after homework time but not directly after use for feeding. Plan of Correction: The school age lead and the site coordinator will work out a plan to sanitize the tables directly after eating before use for homework.
