Comments:
A Subsidy Health and Safety inspection was conducted on 4/5/18 to review supplemental health and safety requirements. An unannounced monitoring inspection was completed on 4/5/18. There were 67 children present in seven groups with 12 direct care staff supervising and two administrative staff in addition were present. Five children's records and five staff records were reviewed. According to the site coordinator, there were no medications at the center for any children in care. The inspector reviewed staff records at the Central site at 10:45 am. The inspection at the facility began at 12:45 pm and ended at 4:30 pm. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on record review and interview, the center failed to ensure that all staff had completed the Virginia Preservice Training for Child Care Staff within 90 days of employment or subsidy vendor approval. Evidence: 1. There was no documentation that staff #3 had completed the required VA Preservice training. Staff #3 date of hire was 9/4/14 and the center has been a subsidy vendor prior to 2017. 2. The child care director verified with the staff that it had not been completed.

Plan of Correction: The staff person will complete the training by the end of next week.
