Comments:
An unannounced monitoring inspection was conducted from 5:00pm-6:15pm. At the time of entrance, 29 children were in care of 2 staff members present. Children were observed during arts and crafts and while engaged in group such as socializing, board games and socializing. A selection of 4 staff and 7 children records, medications and other documents were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-130-A
Description: Based on a sample review of records, 1 child's record was missing documentation of immunizations before the child's first day of attendance. Evidence: The record for Child #1 [date of enrollment: 8/28/17] was missing documentation of immunizations.

Plan of Correction: I will obtain the record from the parent.

Standard #: 22VAC40-185-140-A
Description: Based on a sample review of records, 1 child's record was missing documentation of a physical examination within 30 days of enrollment. Evidence: The record for Child #2 [date of enrollment: 8/28/2017] was missing documentation a physical examination.

Plan of Correction: I will obtain the record from the parent.
