Technical Assistance:
An exit interview was conducted with the director to discuss the results of today's inspection.

Comments:
An unannounced monitoring inspection was conducted today, April 5, 2018 to determine the center's compliance with licensing standards. The inspection was initiated at 2:27pm and concluded at approximately 5:10pm. Upon arrival, the following ratios were observed:

Infant room: 3 children/ 1 staff
Toddler classroom: 6 children/2 staff
Two year old classroom #1: 6 children/2 staff
Two year old classroom #2: 6 children/ 1 staff
Three-Four year old classroom: 18 children/ 2 staff
School age classroom: 6 children/ 1 staff

Children were observed interacting with the teachers, using the restroom, coloring pictures, and playing outdoors. All areas used by children were inspected to include the main hallways, restrooms, classrooms, and common areas. The center's outdoor playground and toys were inspected with no hazards found.

The center had the required postings displayed on the wall and bulletin board upon entering the center and in children's classrooms. The center's first aid kit and emergency supplies were inspected with the required supplies found. Ten children's records and four staff records were inspected with the required information present. The following was also inspected on this day: annual fire & health inspections, fire/shelter-in-place drills, medication/authorizations, parent agreements, and other pertinent information.

Standard #: 22VAC40-185-60-A
Description: Based on the inspection of children's records, the required information was not present. Evidence: During the inspection of ten children's records, it was discovered child #7 did not have the address of the first emergency contact listed. Licensing standards require the full address and telephone number of each emergency contact.

Plan of Correction: Corrected on site.

Standard #: 22VAC40-185-550-D
Description: Based on the inspection of administrative records, the required shelter-in-place drill was not practiced. Evidence: During the inspection of administrative records, it was discovered the center only practiced one shelter-in-place drill for the year 2017. Licensing standards require a minimum of two shelter-in-place drills to be practiced each year.

Plan of Correction: Per director: We will start conducting the minimum two shelter-in-place drills per year.
