Comments:
An unannounced monitoring inspection and facility tour was conducted from 9:50am-10:55am. At the time of inspectors' arrival there were 6 infants in care with 2 staff members. Children were observed participating in infant activities, interacting with staff, feeding, transitioning between activities, and nap. A sample of 5 children's records and 2 staff records were reviewed. Medications were also reviewed. Areas of noncompliance are detailed in the violation notice and were discussed with the assistant director at the conclusion of this inspection.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review it was determined that the center did not ensure that each child's record contained all of the required components. Evidence: The record for child #5 did not contain proof of identification and age.

Plan of Correction: The staff will request proof of identification and age from parent at pick up.

Standard #: 22VAC40-185-510-A
Description: Based on medication review and interview it was determined that the center did not ensure that written authorization was acquired before administering medication. Evidence: The medication authorization form for child #4 did not contain parental authorization/signature. Staff reported that they had been applying the prescription ointment since 4/2/18.

Plan of Correction: Staff will request parent sign the medication authorization form at pick up.

Standard #: 22VAC40-185-510-L
Description: Based on medication review and interview it was determined that the center did not ensure that a record was kept for all medication given to children. Evidence: There was no record of the medication administration for child #4. Staff reported they have administered medication since 4/2/18.

Plan of Correction: Staff will ensure that a medication administration form is completed for all medications administered to children in the center.

Standard #: 22VAC40-185-540-E
Description: Based on supply review and interview it was determined that the center did not have a working battery operated radio. Evidence: There were no batteries for the radio at the center. The staff confirmed that there were no batteries for the radio.

Plan of Correction: The center will purchase batteries for the radio.

Standard #: 22VAC40-185-550-E
Description: Based on record review and interview it was determined that the center did not maintain a record of the dates of the practice drills for one year. Evidence: The record of drills did not contain record of the drills completed prior to March 2018. The staff confirmed that the records for the prior months were not available.

Plan of Correction: The center will maintain a record of all drills.
