Comments:
An unannounced monitoring inspection was conducted beginning at 4:00 and ending at 5:30. There were 43 children with 3 staff. Children were observed at arrival, playing outside, having snack and at Homework time. Arrival was organized and students were observed to be orderly. Reviewed medications for 10 children - see violation notice Reviewed 9 children's records - see violation notice. Reviewed 5 staff files - complete Emergency drills were up to date. Standard #: 22VAC40-185-60-A
Description: Based on a review of 10 children's records, it was determined that the file for 1 child could not be located and the files for two children were missing agreements. Evidence: The record for Child #7 could not be located and the records for Child #3 and Child #6 did not have the parent agreements (Parent Pledge of Understanding)

Plan of Correction: The Director will have the parents sign the agreements this week.

Standard #: 22VAC40-185-510-C
Description: Based on a review o fthe medications for 10 children. it was determined that 1 child's medication was not current. Evidence: The emergency medication for Child #5 , has an expiration date on the medication of 2/18.

Plan of Correction: The Director will notify the parent that a new medication needs to be brought to the center.
