Discussed with center management to assess maintenance issues that may arise, medications.
An unannounced monitoring inspection was conducted from 9:30 am-1:30pm. 71children were in care with 13 staff members present. Children were observed during outdoor play time, group activities, circle time, and lunch. The lunch menu included fish nuggets, roasted potatoes and mandarin oranges with milk. A selection of staff and children records, medications and other documents were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-140-A Description: Based on record review, each child did not have a physical exam. Evidence : 1. Child # 1's record did not have the physical exam. Child # 1's enrollment date at the center was May 2017. 2. Child #2' record did not have the physical exam. Child # 2's enrollment date at the center was 11/1/17. Plan of Correction: We will get the physical exams from the parent.
Standard #: 22VAC40-185-240-D-1 Description: Based on observation and staff interview, staff did not have the required medication administration training to administer medication. Evidence: 1. Center had an Auvi-Q on site for child # 2. 2. There is not staff on site that has obtained the required training for the administration of the Auvi Q. Plan of Correction: We will obtain the required training.
Standard #: 22VAC40-185-270-A Description: Based on observation, areas inside and outside the building were not maintained in a clean, safe and operable condition. Evidence: 1. Two bathroom ceiling tiles in the girl's bathroom were not in place exposing the pipes and leaving an open space. 2. The screens for the windows in the Pre-K and Two's classroom were broken. Some screens were missing and need to be replaced. 3. Areas of the Two's classroom were not clean. There was visible dirt and debris on the carpet area and around the diaper changing table. Plan of Correction: WE will get everything fixed and submit a work order.
