A Subsidy Health and Safety inspection was conducted from 3:28 PM to 6:10 PM for the purpose of reviewing supplemental health and safety requirements. Today, there were twenty-seven children, ages five through eleven years, supervised by three to four staff after school. I viewed compliance with operational responsibilities, record keeping, children's information, staff training, hazardous substances, behavior guidance, hand washing, emergency drills, emergency planning, food allergy information, nutrition and food service, four staff records and five records for children. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. A violation was found during the supplemental subsidy health and safety inspection and is identified on the violation notice. Let me know if you have any questions. 540/430-9259
Standard #: VENDSUB-000-004-F Description: Based on a review of children's allergies and dietary restrictions, an interview with administrative staff, and a review of the record of child 1F, the center failed to post a current list of all children's food allergies. Evidence: Staff indicated child 1F had a food allergy and this type of food was never served to the children. The record contained information which confirmed the type of food allergy. Staff indicated there was not any posting of information about child 1F that was immediately accessible to staff. Plan of Correction: Administrative staff will place the information about the food allergy for child 1F in a location accessible to staff. The staff will be told where the current information about all children's allergies and dietary restrictions is available for viewing.
