Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted today, April 10, 2018. The on-site inspection was from 11:15 a.m. to 1:50 p.m. On this date, there were ten children present and in the care of two staff members. There were a total of 17 children enrolled in the program. The violations from the previous inspection was reviewed for compliance. The exceptions to compliance were noted on the Violation Notice and the risk ratings for the violations were noted on the Supplemental Page. There were five children's records reviewed and two staff records were reviewed for updates. There were two staff members interviewed, as well as three children. There were no medications on-site or being administered. The children brought their lunches from home. The center had just finished a unit on transportation and a fire truck was at the center for the children to see. Some of the items observed and reviewed were: required posted information, documentation of emergency drills, injury reports, current health and fire inspections, first-aid kit and emergency supplies, warm water temperature, outdoor play, art activities, making of Biscuit puppets, bathroom procedures, diapering procedures, story time, lunch, and dismissal procedures. Violations:
Standard #: 22VAC40-185-560-G
Description: Based on observation and confirmation from the director, the center failed to label three lunch boxes with child's name and the date. Evidence: 1. The lunch boxes/bags of three children that had brought their lunches from home were not labeled with the child's name or the date. 2. The director confirmed that the lunch boxes were not labeled with the children's names or the dates.

Plan of Correction: The staff will make sure that each morning the lunch boxes are label with names and dates.

Standard #: 63.2-1809-A
Description: Based on review of five children's records and verification from the director, the center did not obtained proof of one child's identity and age. Evidence: 1. The record for Child C did not contain documentation that the parent had provided proof of the child's identity and age. Date of enrollment for Child C was 08/21/2017. 2. The director confirmed that she had not obtained proof of the child's birth.

Plan of Correction: The director will speak with the parent of the child and will obtain proof of the child's identity and age within seven days or will call local law enforcement.
