Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on April 9, 2018 from 2:30 p.m. through 5:00 p.m. There were thirty-one children in the care of five staff members. Six children's and five staff records were reviewed and updated. No medication is being administered at the present. Areas observed were: arrival, hand washing/bathroom procedures, snack, homework, group activities, and games. Interactions between the staff and children were observed as positive and the children seem comfortable with their daily routine and schedule. Standard #: 22VAC40-185-60-A Description: Based on record review the center failed to obtain the physical address of two emergency contacts for one child. Evidence: 1. Six children's records were reviewed. 2. The physical address for two emergency contacts was not documented for Child #3. 3. The director confirmed the physical address was missing for one child. Based on record review the center failed to obtain the physician's name and address for one child. Evidence: 1. Six children's records were reviewed. 2. The physician's name and phone number was not documented for Child #1. 3. The director confirmed the physician's name and phone number were not documented Plan of Correction: I will request the parent provide two emergency contact addresses for Child #3. I will request the physician's name and phone number for Child #1.
