Comments:
An unannounced Monitoring inspection was conducted this date. A total of 99 children with 15 staff on site during the inspection. Inspection walk thru of all 8 classrooms was completed which also included the school age. The classrooms were decorated with the children's artwork on display such as dot stamp flowers, sensory boards which identified hands, mouth, ears and eyes. One classroom had a theme board about Under the Sea and was decorated with octopus made out of breakfast cereal for tentacles. Observation of transitions from indoor morning play to outside playground was made. Face to name procedures were conducted. Hand washing observed when the children returned from outside play and preparation for lunch was made. Lunch today was beans and rice. Some staff interviewed regarding center emergency procedures, medication needs of the children and food allergies. A total of 10 children's records and 5 staff records to include new hires were reviewed and determined complete. All medications and all topical ointments were reviewed. Effective dates and authorizations were also found complete. This inspection began at 10am and ended at 1:15pm. Thank you for your cooperation today. Please email me at: lisa.hudson@dss.virginia.gov

Description: Based on observation of the infant classroom, the diapering surface was determined no longer non-absorbent. Evidence: The diaper surface located in the infant classroom has a crack in the padding and is no longer non-absorbent.

Plan of Correction: We will replace the diaper pad this week.
