An unannounced monitoring inspection was conducted on April 17, 2018. At arrival to the facility, there were 30 children with four staff members. An additional staff person arrived on site for administrative and support purposes. The children were observed arriving to the program, during music and movement, doing group exercise, having storytime and playing outdoors. Infants were observed napping and during floor and tummy time and crawling freely. Licensing Inspector reviewed supervision, programs, activities, equipment, indoor and outdoor areas, children and staff records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, emergency supplies and required postings. Five children's records and five staff records were reviewed. The center is currently not administering medication. The exit interview was conducted with the Owner.
Standard #: 22VAC40-185-140-A Description: Based on record review, in one of five children's records reviewed, the licensee did not ensure that each child shall have a physical examination by or under the direction of a physician: before the child's attendance or within one month after attendance. Evidence: During the inspection conducted on April 17, 2018, there was no documentation of a physical examination in the record for Child #2 (start date 6/27/16). Plan of Correction: Will have the parents to give a new copy of the physical.
Standard #: 22VAC40-185-160-C Description: Based on record review, in two of five staff records reviewed, the licensee did not ensure that staff resubmit TB test results every two years. Evidence: During the inspection conducted on April 17, 2018, the TB test results for Staff #1 and Staff #2 were more than two years old and were dated 11/23/15. Plan of Correction: Staff #1 will get a new TB test. Staff #2 will bring in a copy of the most recent TB test results.
Standard #: 22VAC40-185-60-A Description: Based on record review and staff interview, the licensee did not ensure that the center would maintain and keep at the center a separate record for each child enrolled. Evidence: During the inspection conducted on April 17, 2018, according to the owner, there was not an established record to review for Child #6. Plan of Correction: We will be locating the child's record and if not a new file will be created.
Standard #: 22VAC40-185-330-B Description: Based on observation and inspection of the facility, the licensee did not ensure that required fall zones shall be underneath and surrounding equipment that requires a resilient surface. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use. Evidence: During the inspection conducted on April 17, 2018, the height of the swing beam for the outdoor spider web swing and the giant saucer spinner tree swing measured approximately 7 feet and required at least a 14 foot fall zone in front and back. The front fall zone was inadequate and measured approximately 11' 1" to the wooden border. Plan of Correction: Owner will submit an allowable variance for the fall zone in front of the equipment.
Standard #: 22VAC40-185-350-E-3 Description: Based on observation and inspection of the facility, the licensee did not ensure that for ratios were maintained for two year old children: one staff member for every eight children. Evidence: During the inspection conducted on April 17, 2018, the following was observed in the Preschool Building: 1. Upon arrival to the center at approximately 9:15am, there were two staff members present with 21 children ranging in age from 2 1/2 years old through five years old. 2. At approximately 9:20am and 9:31 am two additional children arrived. A third staff member arrived at approximately 9:40am. Plan of Correction: Upon staff calling out making substitutes available more quickly. Staff will call earlier when needing to be off.
Standard #: 22VAC40-185-380-A Description: Based on observation and inspection of the facility, the licensee did not ensure that there shall be a posted daily schedule that allows for flexibility as children's needs require. Evidence: During the inspection conducted on April 17, 2018, there was not a daily schedule posted in the Two Year Old classroom. Plan of Correction: A daily routine will be printed and posted today.
