Comments:
An unannounced monitoring inspection was conducted on April 17, 2018 from approximately 9:50 am to 1:10 pm. Ten staff were present with sixty-six children. Additional staff were on site for administrative and kitchen duties. Six children's records and five staff records were reviewed.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review, facility did not ensure 1 of 6 children's records has a physical examination within a month of beginning attendance. Evidence: On April 17, 2018, the record for child #3 (reported start date March 5, 2018) does not have a physical.

Plan of Correction: Parent will be given until the end of next week to provide a physical.

Standard #: 22VAC40-185-150-B
Description: Based on review, facility did not ensure 1 of 6 children's records physical and immunization is signed and dated by the physician, designee or an official of the local health department. Evidence: On April 17, 2018, the record for child #4 has a physical and immunization that is not signed and dated.

Plan of Correction: Parent will provide information.

Standard #: 22VAC40-185-60-A
Description: Based on review, facility did not ensure 3 of 6 children's records have documentation of required information. Evidence: On April 17, 2018, the following was found during record review: 1. The record for child #2 does not have start date and parents' employment phone numbers. 2. The record for child #3 does not have start date and father's information (home and employment). 3. The record for child #4 does not have start date, father's employment phone number and physician's information.

Plan of Correction: Information will be available for parents to update.

Standard #: 22VAC40-185-70-A
Description: Based on review, facility did not ensure 3 of 5 staff records have documentation of required information. Evidence: On April 17, 2018, the following was found during record review: 1. The record for staff #1 does not have start date of employment and emergency contact information. 2. The records for staff #2 and #5 do not have emergency contact address.

Plan of Correction: That information will be updated today.

Standard #: 22VAC40-185-240-A
Description: Based on review, facility did not ensure 1 of 5 staff have documentation of required training within the first date of assuming job responsibilities. Evidence: On April 17, 2018, the record for staff #1 does not have documentation of orientation training.

Plan of Correction: Orientation training will be completed and filed today.

Standard #: 22VAC40-185-240-C
Description: Based on review, facility did not ensure 1 of 5 staff complete at least 16 hours of annual training. Evidence: On April 17, 2018, the record for staff #2 (start date December 27, 2011) has documentation of completing 10 hours of training in 2017.

Plan of Correction: All trainings will be completed and in accordance to hours needed for staff.
