Standard #: 22VAC40-185-60-A Description: Based on file review center failed to maintain an updated record for each child. Evidence: Review of children's records:: child #1, #2 and #3 did not have documentation of parental annual update., last updates were 9/4/16, 12/3/13 and 9/1/2016 respectively. Plan of Correction: Center staff will contact each parent and have them update their files.
Standard #: 22VAC40-185-270-A Description: Based on observation areas of the center shall be maintained were not in a clean, safe and operable condition. Evidence: 1. Six ceiling tiles were observed to be water stained. 2. Peeling paint was observed on the wall located by the children's bathroom. Plan of Correction: Center staff contacted maintance on April 11, 2018 to replace the ceiling tiles, they will follow up regarding the peeling paint and ceiling tiles.
Standard #: 22VAC40-185-510-N Description: Based on medication review, it was observed 2 medications were not returned to parents within 14 days of expiration. Evidence: Epi pen medication for 2 children expired 1/18 and authorization forms expired 6/17. Plan of Correction: Center Supervisor stated he sent out numerous email's telling the parents to come pick up the medications. He will send one last email, if they do not respond, he will take the medication to dispose of.
Standard #: 22VAC40-185-550-C Description: Based on observation shelter in place location was not designed on the posted map. Evidence: Shelter in place location was not noted on the posted maps. Plan of Correction: Center staff will update the map with shelter in place information. Corrected during inspection.
