Technical Assistance:
Allergy plans were reviewed.

Comments:
A Subsidy Health and Safety inspection was conducted on 4/17/2018 from 10:00 a.m. until 1:30 p.m. to review supplemental health and safety requirements. There were 14 infants and toddlers in care with staff supervising at all times. Diapering, playing outside, being fed, and napping were observed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Description: Based on record review and observation, there was not a written allergy plan for a child with a diagnosed food allergy. Evidence: 1. The inspector reviewed five children's records. There is one child (file # 5) in care with a food allergy and there was not an allergy plan in the file. 2. The director confirmed there was not an allergy plan.

Plan of Correction: The director will work with the guardians to get a completed allergy plan.

Standard #: VENDSUB-000-012-B
Description: Based on record review, all staff had not completed the VA Preservice Training within the first 90 days. Evidence: 1. The inspector reviewed 5 staff records. One of five staff (#2) had not completed the VA Preservice training.

Plan of Correction: The staff person will complete the training.
