A Subsidy Health and Safety inspection was conducted on 4/16/18 to review supplemental health and safety requirements. An unannounced monitoring inspection was completed on 4/16/18. There were 106 children present in seven groupings with 16 staff directly supervising, a kitchen staff person, and two administrative staff to assist with supervision as necessary. Ten children's records and five staff records were reviewed. Four additional staff records were reviewed to observe background checks only as they were newly hired staff. Two over-the-counter, as needed medications were reviewed. The inspector arrived at the center at 9:00 am and departed at 2:15 pm. The children were observed during free play times in the classrooms, during movie times, during preparation for lunch, during lunch, and during nap. The school-age children were present due to schools being closed because of the inclement weather/natural disaster after-effects and their activities to include lunch and movie time were observed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on record review, observation, and interview, the center failed to ensure that a child's food allergy was posted. Evidence: 1. The physical for child #10 indicated that the child had a diagnosed food allergy to eggs. 2. The list of allergies, intolerances, and preferences that was posted in the cafeteria did not include this allergy. 3. The director did confirm this child's food allergy and stated that it was not posted because the center does not serve eggs.

Plan of Correction: This was corrected during the inspection. The director posted this (there was written parent permission to post another allergy for this child in the child's record).

Description: Based on record review and interview, the center failed to ensure that there was a written allergy care plan for each child with a diagnosed food allergy. Evidence: 1. The record of child #10 was reviewed. There was a diagnosed food allergy for eggs. There was no written allergy care plan to include steps to be taken in the event of a suspected or confirmed allergic reaction. 2. The director stated that the center does not serve any eggs and therefore, the child did not have a written allergy action plan.

Plan of Correction: The director will request a written allergy action plan to be completed by the child's physician.
