Standard #: 22VAC40-185-140-A Description: Based on record review, children's records did not contain all required information. Evidence: Child #5 first date of attendance was 8/30/2017 and their file did not contain a physical exam. Plan of Correction: We will contact the parents by email today.
Standard #: 22VAC40-185-510-C Description: Based on record review and observation, the center did not follow procedures for preventing the use of outdated medication. Evidence: Child #2 emergency medication expired 1/2018. Child #5 emergency medication expired 3/2018. Both medication had not been returned to the parents. Plan of Correction: We will inform the parents right away by phone.
Standard #: 22VAC40-185-510-G Description: Based on observation, prescription medication was not kept in it's original labeled container. Evidence: Child #1 emergency medication is stored in a ziplock bag without the original box or prescription label. Plan of Correction: We will inform the parents right away by phone.
Standard #: 22VAC40-185-510-N Description: Based on record review and observation, medication was not returned to parents within 14 days of the medication authorization expiring. Evidence: The short-term medication authorization (signed by parents only) for Child #3 emergency medication expired on 1/19/2018. The short-term medication authorization (signed by parents only) for Child #4 emergency medication expired on 9/25/2018. The long-term medication authorization (signed by parents and physician) for Child #5 expired 3/5/2018. All medications had not been returned to the parents. Plan of Correction: We will inform the parents right away by phone.
