22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was conducted today from 9:40am-2:45pm. There were 62 children (infants-5yrs.) directly supervised by 18 staff. The physical plant, 6 staff records, 6 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories being read to them, writing numbers, painting using watercolors and chalk, napping, being bottle fed, singing, and eating lunch. Today's lunch consisted of Turkey sausage patty, whole-grain biscuit, green beans and fresh orange slices. Diapering and hand-washing procedures were also observed. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Description: Based on record review, the center did not obtain immunization documentation for a child before the first day of attendance. Evidence: Child#5's record did not have immunization documentation before the first day of attendance. The child's first day of attendance was documented as 04/09/2018.

Plan of Correction: Corrected.

Standard #: 22VAC40-185-60-A
Description: Based on record review, children's record were incomplete. Evidence: Child#1's record is missing both parents' work telephone numbers. Child#2's record is missing a complete address for both emergency contacts. Child#4's record is missing the mother's workplace and work telephone number, and the father's work telephone number, child#5's record is missing the father's workplace and work telephone number.

Plan of Correction: We are updating our emergency contact form to include parents' workplace and work telephone number.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas of the center were not maintained in a safe and operable condition. Evidence: In the bathroom located inside the two's classroom, one of the floor tiles has a broken area missing a piece of tile. In the school-age classroom, two small vinyl chairs located in the reading area were observed with rips in the seams, and a wooden toy kitchen in the classroom was observed missing a knob, the screw that held the handle on was still sitting in the opening, and protruded from the hole about 1/8 of an inch.

Plan of Correction: We have a work order in and we will follow up on it.

Description: Based on observation, not all unused electrical outlets had protective covers. Evidence: Two outlets were observed in the hallway without protective covers.

Plan of Correction: Corrected during the inspection.

Standard #: 22VAC40-185-510-A
Description: Based on review of medication, it was determined that a child had two medications on site without a medication authorization form. Evidence: Child#6 has two emergency prescription medications on site. Both of these medications were on-site without a medication authorization form completed and signed by the parent and the physician.

Plan of Correction: Form was given to parents to fill out and get signed by the doctor.

Description: Based on observation, a bottle of milk brought from one of the children's home was not labeled with the child's name and the date. Evidence: In the two's classroom a bottle of milk was observed on top of a shelf. The bottle was brought from home, without a label identifying the child's name and date.

Plan of Correction: We provided labels to the parents to appropriately label the bottles each day.

Description: Based on record review, a staff member did not have documentation of a sworn statement or affirmation prior to their first day of employment. Evidence: The record of staff#1 did not contain a sworn statement or affirmation form. Staff member's first day of employment was documented as 12/11/2017.

Plan of Correction: Corrected during the inspection.

Standard #: 63.2-1720.1-B-2
Description: Based on record review, a staff member did not have documentation of a criminal history check prior to their first day of employment. Evidence: Staff#1 did not have documentation of a criminal history check on file. Staff member's first day of employment was documented as 12/11/2017.

Plan of Correction: Staff member was scheduled to get the background check the following day.
