An unannounced monitoring inspection was conducted today from 9:30am-12:15pm. There were 32 children (16 month-5yrs.) supervised by 5 direct care staff. The physical plant, 4 staff records, 3 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. There were no medications present at the time of inspection. Children were observed eating breakfast, transitioning to circle time, singing, drawing self-portraits, playing with puzzles, and engaging in outdoor play activities . There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-60-A Description: Based on record review, all children's records did not contain all of the required information. Evidence: 1. Child #1, Child #2, and Child #3's records did not contain complete parent work information, to include parent work location and phone numbers. 2. Child #1 (start date: 6/20/2016) and Child #3's (start date: 9/8/2016) records did not have documentation of an annual review of their record. Plan of Correction: I will get in touch with the parents today, and will have the information fixed by next Tuesday.
Standard #: 22VAC40-185-70-A Description: Based on record review, all staff records did not contain all of the required documentation. Evidence: 1. Staff B's (start date: 4/11/2012) record did not contain documentation of the individual's level of education. 2. Staff D's (start date: 10/5/2015) record did not contain documentation of 2 required reference checks. Plan of Correction: I will have Staff B bring documentation of his education by Monday. I will contact Staff D's references today.
