Comments:
A Subsidy Health and Safety inspection was conducted on 4/20/2018 to review supplemental health and safety requirements. There were 32 children supervised by 5 direct care staff. The physical plant, 4 staff records, 3 children's records, evacuation drills, injury reports, emergency supplies, and policies were inspected. There were no medications present at the time of inspection. Children were observed eating breakfast, transitioning to circle time, singing, drawing self-portraits, playing with puzzles, and engaging in outdoor play activities. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: VENDSUB-000-012-B
Description: Based on record review, all employees did not have documentation of completed Virginia Preservice Training within the first 90 days of employment. Evidence: Staff A's (date of hire: 9/13/2017) record did not have documentation of completing the Virginia Preservice Training.

Plan of Correction: I will have her complete the training before she comes back to work.

Standard #: VENDSUB-000-032-A
Description: Based on record review and staff interview, all emergency response drills were not conducted as required. Evidence: At the time of inspection, there was no documentation of an annual lock down drill being conducted. Director stated that one was not conducted within the past year.

Plan of Correction: I will include the lock down drill in my drills, and do one next week.
