An unannounced monitoring inspection was completed on April 20, 2018. The purpose of this visit was to measure compliance with the licensing standards. There were 24 children, toddlers and preschool age, in care at the time of the inspection. There were five staff members supervising the children in their classrooms. All groups were in proper ratio. Nap time was observed. All areas of the center including classrooms, hallways, bathrooms and playgrounds were inspected. The facility was equipped with manipulatives and supplies, and items were available to the children. The following information was reviewed: fire and shelter-in-place drills, liability insurance policy, parent agreements, and staff training information. Only emergency medication is administered. The merry go round equipment on the play area is not in use.

During the inspection, five children's records and five staff staff records were reviewed.

Standard #: 22VAC40-185-140-A Description: Based on observation the center did not ensure that each child shall have a physical examination by or under the direction of a physician. Evidence: The Licensing Inspector reviewed five children's records. Child #1 did not contain a physical examination record. Plan of Correction: The center director stated that child #1's mother will be notified today and the physical record will be obtained by Wednesday April 25, 2018.
Standard #: 22VAC40-185-270-A Description: Based on observations at the facility, the facility did not ensure that all areas and equipment, inside and outside of the facility,were maintained in a safe condition. Evidence: 1. The fence surrounding the playground is broken in three areas. There were 18 rusted and protruding nails and screws. 2. The vent frame located in the back hallway has two protruding sharp metal edges. Plan of Correction: The center director will have a maintenance person fix the fence and vent frame by next Wednesday 04/25/2018.
Standard #: 22VAC40-185-280-B Description: Based on observation the center did not ensure that hazardous substances and other harmful agents were kept in a locked place using a safe locking method that prevents access by children.
The Licensing Inspector observed:
1. Hand sanitizer with a label that states "Keep Out of Reach of Children" was located on the arts and craft shelf in the preschool classroom. The hand sanitizer was in the reach of children.
2. Shaving cream with a label that states "Keep Out of Reach of Children" was located on the arts and craft shelf in the preschool classroom. The cream was in the reach of children.
Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-185-290-3 Description: Based on observation the center did not ensure that electrical outlets shall have protective covers that are of a size that cannot be swallowed by children. Evidence: The Licensing Inspector observed two extension cord outlets without protective covers. The outlets were located under the television in the preschool classroom. Plan of Correction: Corrected during the inspection.
