Technical Assistance:
Consultation was given on the new background requirements for finger printing, a revised Sworn Statement (SDS) for new staff hired after 7/1/2017 and requesting out-of-state Central Registry checks for staff who have lived outside of Virginia within the last five years. The model SDS form is online at VA DSS as well as a link to the Adam Walsh site to assist with out-of-state central registry forms. VA DSS will be paying for initial child care finger print checks until 9/30/2018. Each facility must establish a point of contact with VA DSS. All questions regarding facility codes should be directed to OBI at 804-726-7884. Eligibility letters will be sent out by e-mail and staff may not be hired until the completed letter is received. Please read, review, and respond to all correspondences regarding the fingerprint roll out. Discussed maintaining up to date staff records including background checks. Staff records must be available on site for review during inspection.

Comments:
An unannounced monitoring inspection was conducted between 7:50am to 9:45am. There were 13 children in direct care of 2 staff members. The children were observed getting ready to transition to school. 4 staff and 5 children's records were reviewed. There were no medications on site. Emergency drill records and first aid kit was reviewed. Areas of non compliance are discussed in the violation notice. Violations:
Standard #: 22VAC40-185-160-A
Description: Based on record review, each staff member did not obtain the Tuberculosis (TB) Screening within 21 days after employment. Evidence: Staff # 1 was hired on 2/16/18. There was no record a negative TB screening in her record.

Plan of Correction: I will get the TB screening and place in staff's file.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, center did not have the central registry findings for staff within 30 days of employment. Evidence: 1. Staff # 1 and #2's records did not have the central registry findings. 2. Staff # 3 stated that the results have come in from the agency and were in the mail. 3. The results for staff # 1 and # 2 were not available on site for review.

Plan of Correction: We will place the results in the staff files.

Standard #: 63.2-1720.1-B-2
Description: Based on record review and staff interview, center did not obtain fingerprinting results for staff. Evidence: 1. Staff # 1 was hired on 2/16/18. 2. Staff # 1's record had manual fingerprinting results from the VA State Police. Fingerprinting was not done through the state approved vendor.

Plan of Correction: I will talk to center management to get fingerprinting done.
