Evidence: During the inspection on April 18, 2018, there was not a written record for the 15 children in the 3's and 4's room and a complete record for the children in the 2's room as there was only documentation of the arrival of 9 of the 13 children in care. Plan of Correction: The center director will retrain the staff to maintain the log as it happens.
