A Subsidy Health and Safety inspection was conducted on April 23, 2018 to review supplemental health and safety requirements. Upon arrival at 10:35 AM there were a total of twenty-six (26) children ages 7 months-4 years old were present under the direct care & supervision of five (5) caregivers. Licensing Inspector was unable to review staff and children records; records were requested but not provided. Children were observed during arrival/departure procedures, outdoor play, rest/sleeping activity, bathroom/hand washing procedures, snack/meal served, group and self-initiated activities. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: VENDSUB-000-006-A Description: Based on inspection, the Vendor failed to maintain a record for each enrolled child and make accessible to the department's representative. EVIDENCE: Children records were not made accessible to the Licensing Inspector. Licensing Inspector requested to review Center records upon arrival and was not provided with the records during the inspection. Plan of Correction: Center Director will review all records with the administrator.
Standard #: VENDSUB-000-028-A Description: Based on observations, the Vendor failed to ensure staff wash their hands with soap and running water before and after diaper change. EVIDENCE: In the infant classroom located on the 1st floor, staff was observed during diapering several infants in care. Staff did not wash their hands before/after each diaper change. Plan of Correction: Staff will receive additional training on diapering/hand washing procedures
Standard #: VENDSUB-000-028-B-6 Description: Based on observations & discussion with staff, the Vendor failed to ensure disposable diapers are disposed in a leakproof or plastic-lined storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. EVIDENCE: In the infant classroom located on the 1st floor, staff was observed during diapering several infants in care. Staff used her hands to open/close trash can to dispose of the soiled diapers. Foot-operated trash can was available however the foot pedal was not used to open/close the trash can. Staff further disclosed that soiled diapers containing fecal matter is wrapped in a plastic bag and immediately taken to the trash can located outside the classroom which requires using staff to use their hands. Plan of Correction: Staff will use foot pedal on the foot operated trash can to dispose of all soiled diapers.
