22VAC40-191 Background Checks (22VAC40-191)

An unannounced renewal inspection was conducted today, April 20, 2018 to determine the center's compliance with licensing standards. The inspector arrived at 1:30 pm and concluded the inspection at 4:45 pm. Upon the inspectors arrival, children grades 1st through 3rd were at Jefferson Park having field day while children ages two and a half through six years were napping. Later children in the pre-primary class were observed having snack, looking at books and working puzzles. A parent was scheduled to come to read a story later in the afternoon to the class. In primary I, children were preparing for Continent Day, with a focus on North America's Mexico. Children were making pi?atas out of bags and tissue paper and clay pots. Primary II had time outside where they played a group game that originated from Guatemala, as they were studying the continent of Central America. Parent pick up was also observed. Children provide their own meals which were labeled with names. All areas of the facility were inspected. The facility has age and stage appropriate materials and supplies. The facility had the following posted: License, last inspection, emergency numbers, evacuation maps/procedures, daily schedule, allergy information and various parent information. The first aid kit and non-medical emergency supplies were inspected and found complete on both floors of the facility. The playground was inspected (see violation notice). The inspector interacted with staff and children in care. Eight staff records and ten children's records were reviewed during today's inspection. The following ratios were observed in each classroom: Primary I: (3-6 years) 17 children with 2 staff and two parent volunteers. Primary II: (3-6 years) 18 children with 2 staff. Pre-Primary: (2.5-3.5 years) 15 children with 3 staff. The last evacuation drill was conducted on 03/22/2018. The two required shelter-in-place drill for 2018 year was completed on 01/11/2018 and 03/20/2018. A second drill will need to be conducted by the end of the school year. The facility has current liability Insurance. The Emergency Preparedness plan is current. Health Inspection: 02/13/2018 Fire Inspection: 10/09/2017 Medications were reviewed. Multiple staff have current MAT certification. Staff have current CPR & First aide training along with Daily Health Observation training. The violations cited during the licensing period were checked for corrections. There was one repeat violation during today's inspection. Standard #: 22VAC40-185-160-C
Description: Based on review of staff records, the facility did not obtain results of a tuberculosis screening at least every two years from the date of the first initial screening for each staff. Evidence: The most recent tuberculosis screening documentation for staff #1 was dated 02/26/2016, exceeding the two year allowance to renew.

Plan of Correction: Per the administrator: We will have the staff have a test done by Monday and obtain the results by Thursday.

Standard #: 22VAC40-185-330-B
Description: Based on observation and measurement taken, the facility did not ensure where playground equipment is provided, resilient surfacing complied with minimum safety standards. Evidence: 1. The resilient surfacing on one side of the A-frame climber had three measurement of less then three inches of mulch. 2. At the foot of the sliding board shot, dirt was exposed with a measurement of less them an inch of resilient surfacing.

Plan of Correction: Per the administrator: We will order additional mulch and meet with the outdoor committee.

Standard #: 22VAC40-185-550-M
Description: Based on review of injury reports, the facility did not ensure each report contained all required information. Evidence: Multiple injury reports were missing the following required information; date and time when parents were notified and how parent was notified.

Plan of Correction: Per the administrator: We will conduct additional training for staff to ensure they complete all required information on the injury report.
