Comments:
The licensing inspector conducted an unannounced monitoring inspection Tuesday, April 24, 2018 from approximately 2:15PM until 4:40PM. The following ratios were observed with a total of 31 children present during the inspection: Infants - 6 children with 2 staff Toddlers - 5 children with 1 staff Twos - 5 children with 1 staff Threes - 7 children with 1 staff Fours & Fives - 8 children with 1 staff School age children were not present during the inspection. When the licensing inspector arrived at the center, children were observed napping. Later during the inspection, the licensing inspector observed children having their diapers changed, children taking bathroom breaks and washing their hands before eating snack, playing with toys, looking at books and participating in circle time. The following information was reviewed: daily attendance, Infant daily sheets, practiced emergency drill logs, injury reports, parent agreements, menu, required postings, staff training information and children?s assessments. The center's first aid kit and emergency supplies were inspected. A member of management stated there were no medications on site. During the inspection, five children's records and five staff records were reviewed. The licensee must send documentation to the licensing inspector that the background checks have been requested no later than 10 days following this notification dated April 24, 2018. Standard #: 22VAC40-185-340-D
Description: Based on observation and interview, the facility did not ensure each grouping of children had at least one staff member regularly present who met program leader qualifications. Evidence: During the inspection, the licensing inspector observed Staff #1 supervising five children in the Two?s classroom. The licensing inspector reviewed the record for Staff #1 and it was determined that Staff #1 does not meet the program leader qualifications. Staff #1 stated she has been working in the Two?s classroom for the last few weeks.

Plan of Correction: We will work on getting Staff #1 qualified as the lead teacher.

Standard #: 22VAC40-185-350-C
Description: Based on observation, review of documentation and interview, the center did not ensure the required staff-to-child ratio was maintained for the youngest child in the grouping. Evidence: During the inspection, the licensing inspector observed five children being supervised by one staff member in the Toddler classroom. Child #1 was one of the five children present in the Toddler room. Child #1 turned 12 months of age in March 2018. The required staff-to-child ratio is one staff person for every four children, birth to 16 months of age.

Plan of Correction: Corrected during the inspection. Child #1 was brought back to the Infant classroom and will remain there until the child is old enough to be in the Toddler classroom.

Standard #: 63.2-1720.1-A
Description: Based on record review and interview, the facility allowed a staff member to supervise children prior to receiving notice of eligibility based on results of a national fingerprint-based criminal history record check. Evidence: During the inspection, the licensing inspector observed the record for Staff #1 (hire date 04/10/18) did not contain the required eligibility letter stating staff was eligible to work in a child care center after completing a national fingerprint-based criminal history record check. A completed Virginia State Police criminal history record dated 04/09/18 was located in the record for Staff #1. As of 01/22/2018, a statement of eligibility based on a national fingerprint-based criminal history record check is required to be obtained prior to staff working in the facility.

Plan of Correction: We will have the staff member schedule an appointment and go through the fingerprint based criminal history record check.
