22VAC40-191 Background Checks (22VAC40-191)
- Technical Assistance:
During the inspection, the licensing representative discussed with center administrator updates to background procedures. The center has received their facility code and has began fingerprinting per the new guidance. Staff was informed of a child whose record will require updated immunizations in the coming weeks. [Child #3]
An unannounced monitoring inspection was conducted today from 11:15am-2:20pm. A total of 57 children were observed in the care of 12 staff present. During the inspection, diapering, handwashing, infant bottle feeding, transitions from outdoor play and lunch were observed. A total of 8 staff records, 6 children's records, injury reports, emergency drills and medications were reviewed. Areas of noncompliance were noted on the inspection/violation report. Standard #: 22VAC40-185-130-B Description: Based on a review of records, 2 children's records were missing documentation of additional immunizations every six months. Evidence: The most recent immunizations in the record for Child #2 [15 months old] were obtained 9/8/2017; The most recent immunizations in the record for Child #5 [16 months] were obtained 3/29/2017. Plan of Correction: We will request the information from the parents.
Standard #: 22VAC40-185-60-A Description: Based on a review of records, 1 child's record was missing the required information. Evidence: The record for Child #1 was missing proof of identity. Plan of Correction: We will add the information to the record.
Standard #: 22VAC40-185-280-B Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method. Evidence: 1. A cleaning agent was observed on a counter space in an infant room. 2. A lock was not installed in a lower cabinet where cleaning agents were stored the twos room. Plan of Correction: Both were corrected during inspection.
Standard #: 22VAC40-185-280-D Description: Based on observation, cleaning materials were located above food. Evidence: 2 cleaning agents were observed in an unlocked cabinet where food was stored. Plan of Correction: We will remove food from cabinet and make sure the lock is installed.
Standard #: 22VAC40-185-420-E-3 Description: Based on a review of records, 1 child's record was missing an annual update of the required information. Evidence: The most recent update documented for Child #4 was obtained 1/27/2016. Plan of Correction: We will have the parent update the record.
Standard #: 22VAC40-185-520-A Description: Based on a review and observation, requirements for the use of nonprescription topical creams were not met. Evidence: 3 diaper creams were not labeled in an infant room; 5 diaper creams were missing written authorizations from parents in a twos room. Plan of Correction: We will have authorizations completed or topicals sent home.
Standard #: 22VAC40-191-60-C-2 Description: Based on a review of records, 1 staff's record was missing central registry results within 30 days of employment. Evidence: The record for Staff #5 [date of hire: 2/6/2018] was missing results of a central registry check. Plan of Correction: We will add the results to the record.
