Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and a review of changes to background checks with regards to expiration dates, new fingerprint checks to be done through Fieldprint of Virginia & Office of Background Investigations (OBI), new sworn disclosure statements, and to obtain CPS checks from other states as listed from the past five years. The roll out for the new fingerprint checks occurred on January 22, 2018. All finger print checks for existing employees and board officers must be started for each according to the established time table and then completed by September 30, 2018. New employees will not be allowed to start until an ?Eligibility Letter? is on file.

Comments:
The following inspection took place between the hours of 10:30 am through 2 pm. Today, I met with the Staff of Your Child's Place Child Day Center (CDC) located in Vienna, Virginia. At the start of this unannounced monitoring inspection there were a total of 54 children with 11 classroom staff; good ratios in all 8 groups. A total of 5 children?s files, 2 board officers files, and 3 staff files were reviewed. The children were engaged in the following: infant feeding, infant sleeping, diaper changing, art, story time, center play with the classroom, learning, toy play, lunch, nap time, and other organized activities. Standard #: 22VAC40-185-140-A
Description: Based on review, 1 of 5 children's files reviewed indicated that each child did not have a physical examination by or under the direction of a physician within one month after attendance. Evidence - 1. On the date of inspection (04/25/2018), documentation of a physical examination was not available for review for Child #5. 2. The start date of Child #5 was listed as 01/08/2012. 3. While several immunization pages were on file, the page for the physical was left blank.

Plan of Correction: The Parent of Child #5 shall be contacted and a current physical shall be obtained.

Standard #: 63.2-1720.1-B-3
Description: Based on review, 2 of 2 board officer files reviewed indicated that the center did not obtain a copy of the results of any child abuse and neglect registry or equivalent registry maintained by any other state (federal district or U.S. territory) in which the individual has resided in the preceding five years for any possible founded complaint of child abuse or neglect against him or her. Evidence - 1. On the date of inspection (04/25/2018), an out of state equivalent registry check for Board Officer #1 and Board Officer #2 were not obtained by the center through the District of Columbia (DC). 2. Board Officer #1 stated (during an interview conducted on the date of inspection) that both herself (Board Officer #1) and Board Officer #2 currently live outside the Commonwealth of Virginia. 3. Both Board Officer #1 and Board Officer #2 were with the center prior to 07/01/2017 when this code was enacted. 4. DC residency was also indicated on both Searches Against the Central Registry (CPS) conducted through the Commonwealth of Virginia. No findings were reach for either check.

Plan of Correction: A request for a finding from the District of Columbia shall be sent for both Board Officer #1 and Board Officer #2 within 10 days. Proof of this sent request shall be made available to our Licensing Inspector.
