Comments:
A review of staff personnel files was conducted on April 16, 2018 by inspectors: Kimberly Sawyer, Megan Smith, and Pam Sneed at the Campagna Center office building. Area of noncompliance is identified in the Violation Notice.

Violations:
Standard #: 63.2-1720.1-B-3
Description: Based on review of ten staff records, the center did not complete out of state central registry requests of other states in which contracted employees currently reside. Evidence: The sworn disclosure statements completed by staff #1 and #2 indicate other states of current residency. The records for staff #1 and #2 did not have documentation of out of state central registry requests.
