An unannounced monitoring inspection as conducted today after school. There are 65 children with 6 staff. 8 staff (specific items only) and 6 children's records were reviewed. See violation notice for missing information. Medications, emergency supplies, emergency evacuation drills and injury reports are all in compliance. Children were observed washing hands, eating snack, playing on the playground and in the SACC rooms. Standard #: 22VAC40-185-130-A Description: based upon a review of records, one child's record does not contain documentation of an immunization record, prior tothe first date of attendance. Evidence: Child 4's record has no immunization record. (Date of enrollment 9/6/16 Plan of Correction: Shot record was obtained during the inspection from the school office.
Standard #: 22VAC40-185-140-A Description: Based upon a review of records, children's records do not contain documentation of a completed physical within 30 days of enrollment. Evidence: 3 of 6 records reviewed were missing documentation of a physical within 30 days of enrollment. Child 4 - Date of enrollment 9/6/16 Child 1 - Date of enrollment 8/29/17 Child 3 - date of enrollment - 9/6/16 Plan of Correction: Physicals were obtained during the inspection from the school office.
Standard #: 22VAC40-185-540-A Description: Based upon a review of first aid supplies, the first aid kit is incomplete. Evidence: Missing items include: 1 triangular bandage. Plan of Correction: I will make a bandage for the first aid kit.
