Comments:
Thank you for your assistance during this unannounced renewal inspection. This on-site visit was conducted on April 25, 2018 from 10:45 a.m. through 2:30 p.m. Upon arrival, there were seventy-five children in the care of fourteen staff. Seven children's and twenty staff records were reviewed and updated. The medication and medication paperwork was reviewed for one child. Areas of non-compliance may be found on the violation notice. Areas observed were: outside play, free choice activities, hand washing/bathroom diapering procedures, infant feeding, lunch, story-time and nap. Interactions between the staff and children were observed as positive and the children seem comfortable with the daily routine and schedule. Description: Based on record review, the center failed to obtain an out of state central registry check for one employee. Evidence: 1. Twenty staff records were reviewed. 2. Staff # 9 did not have documentation of an out of state central registry check. 3. The director confirmed the out of state central registry search had not been obtained for staff # 9

Plan of Correction: An out of state central registry check will be obtained for Staff # 9.
