The licensing inspector conducted an unannounced complaint inspection in response to a complaint regarding supervision and nutrition that was received by the licensing department on 04/11/2018. The inspection was initiated and completed on 04/17/2018. On 04/17/2018, the inspector was on site from 8:55AM to 10:15AM. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation supports a portion of the allegation; therefore, the complaint was determined to be valid. See the violation notice for additional details. Please complete the 'plan of correction' and 'date to be corrected' for each violation cited on the violation notice and return it to me within 5 business days from date of receipt. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s). Standard #: 22VAC40-185-350-E-4 Complaint related: Yes Description: Based on observations, documentation review and interviews, the facility did not ensure the required staff-to-child ratio of one staff member for every 10 children was maintained for children three years to the age of eligibility to attend public school, five years by September 30, 2018. Evidence: During the inspection, the licensing inspector observed the following: 1. The licensing inspector arrived at the facility on 04/17/2018 at 8:55AM to address a complaint received by the department. 2. At 8:58AM, the licensing inspector observed Staff #1 supervising 13 children alone, in the first classroom located on the right side of the hallway. 2. Staff #1 stated the children in the classroom were three and four years of age. 3. The licensing inspector asked Staff #1 if she was by herself. Staff #1 stated she was by herself since Staff #2 left a few minutes early because she had to get her kids and take them to her class. 4. At 9:04, Staff #3 arrived at the center and assisted Staff #1 supervise the 13 children in the classroom. 5. Staff #3 stated two staff members were absent for the day. The required staff to child ratio for the classroom was one staff member for every 10 children present. Plan of Correction: Not available online. Contact Inspector for more information.
